For your child to attend the Momentum Youth Conference you must fill out Registration, Liability/Release, and Insurance forms for BOTH First Church of God and the Momentum Conference.

You will first complete the Registration and Student Release here, through the First Church site. Once complete, you will receive an email with information, links and video instructions for how to proceed to the second part - registering and paying with Momentum Conference.

Much of the information will be repeated among the forms between First Church and Momentum. Be sure to have your insurance information and your student's prescription medication information ready. Click Next to proceed.
